What Is Surrogacy

Understanding
Gestational Surrogacy

A gestational surrogate — also called a gestational carrier — carries a pregnancy for another person or couple. The baby is created through IVF using the intended parents' embryo (or a donor embryo). You share no genetic connection with the child you carry.

Unlike traditional surrogacy (where the surrogate uses her own egg), gestational surrogacy involves only the transfer of an already-created embryo into your uterus. You are giving the embryo a safe, healthy home for nine months — that is your extraordinary contribution.

The process is medically straightforward for women who have had successful pregnancies. The medications involved are similar to hormone support used in fertility treatments, and the embryo transfer itself is a brief, non-surgical procedure.

Eligibility

Basic Requirements

These are the baseline criteria. Not every situation is black-and-white — if you are unsure whether you qualify, reach out and we will talk through your specific circumstances.

Age 21–40
Must be within this range at the time of embryo transfer
Previous successful pregnancy
At least one uncomplicated delivery; currently parenting that child
Healthy BMI
BMI under 33 at time of application (IVF clinics vary)
Non-smoker & drug-free
No tobacco, vaping, recreational drugs, or marijuana use
US resident
Must reside in the United States; California and other surrogacy-friendly states preferred
Financially stable
No current government financial assistance
No major psychiatric history
No active treatment for schizophrenia, bipolar I, or similar conditions
Supportive home environment
Partner or family support during the pregnancy
No nicotine or antidepressants
Must be off antidepressants for at least 6 months prior to transfer
Willing to follow medical protocols
Injections, monitoring appointments, and IVF medications are required
Not sure if you qualify? Many of the requirements above can be discussed case by case. Past cesarean sections, gestational diabetes, or mild antidepressant history do not automatically disqualify you. The best way to find out is to submit an application and speak with our team.
The Process

From Application
to Match

Our screening process typically takes 6–10 weeks. We move at a pace that is comfortable for you.

Submit Your Application

Fill out our online surrogate inquiry form. Our team reviews every application within 2 business days. If you appear to meet our basic criteria, we will reach out to schedule a phone screening.

Phone Screening with a Coordinator

A member of our surrogate team will call you for a relaxed, confidential conversation. We want to hear your story, understand your motivations, and answer your questions. This is not an interview with right or wrong answers — it is simply a chance to get to know each other.

Medical Records Review

We request your obstetric records from previous deliveries and any relevant medical history. Our team, along with a consulting IVF physician, reviews your records for medical pre-qualification. We handle obtaining the records directly from your providers.

Social Screening Video Interview

A face-to-face video call with your assigned coordinator. We get to know your home situation, discuss your support system, explore your motivations more deeply, and cover what expectations will look like during the journey. This helps us find you the best possible match.

Partner or Spouse Meeting (if applicable)

If you have a partner or spouse, we schedule a separate video call to ensure they fully understand and support the surrogacy process. A strong support system at home is one of the most important factors in a successful journey.

Background Check & Welcome Packet

You and your partner (if applicable) complete a background check. We send your welcome packet with all the paperwork needed to move forward, including identification documents and income information used to calculate any lost-wages coverage.

Medical Pre-Screening

Before matching, we complete a uterine lining assessment — either a mock cycle with your local OB or a baseline ultrasound depending on your contraception method. This confirms your uterine lining is adequate and screens for fibroids, polyps, or other concerns before you are presented to intended parents.

Psychological Evaluation

A licensed mental health professional specializing in third-party reproduction conducts an evaluation — typically a video call plus a written assessment (MMPI-2). This is standard for all surrogacy programs and is designed to ensure you are emotionally prepared, not to disqualify you.

Matching with Intended Parents

Once you have cleared all screening steps, we begin the matching process. You receive a "Dear Surrogate" letter from potential intended parents along with their profile. When both you and the intended parents feel excited about the match, we arrange an introductory video call. You always have the right to decline a match.

Access to surrogacy-experienced therapists and counselors throughout your journey — not just during the evaluation phase.

Legal Representation

You have your own reproductive attorney — separate from the intended parents' attorney — to review and negotiate the Gestational Surrogacy Agreement.

Insurance Review

We review your existing health insurance for surrogacy coverage. If your policy excludes surrogacy, we coordinate a supplemental maternity policy at no cost to you.

Surrogate Community

Connect with other Alpha Fidelis surrogates — past and present. Our community events and private groups provide connection with people who truly understand your experience.

Compensation

You Deserve to Be Well Compensated

Surrogate base compensation at Alpha Fidelis ranges from $40,000 – $65,000, paid in monthly installments throughout your journey. In addition to base pay, you receive allowances and bonuses that cover every expense related to the surrogacy — you should never pay anything out of pocket.
